ACUTE CARE EPISODE (ACE) DEMONSTRATION ACKNOWLEDGEMENT

The Acute Care Episode (ACE) Demonstration is a new demonstration that will test the use of a bundled payment for both hospital and physician services for a select set of inpatient episodes of care for cardiovascular procedures.

You may be eligible for this demonstration if you are admitted as an inpatient and have one of the following procedures:

Cardiovascular Procedures:
Coronary Artery Bypass Graft Surgery
Cardiac Valve Replacement Surgery
Cardiac Pacemaker Implantation and Replacement
Cardiac Defibrillator Implantation
Coronary Artery Angioplasty

If you develop other diagnoses or have other significant procedures performed this may eliminate you from this demonstration.

If you meet the above requirements for the demonstration then you will be provided an opportunity for Medicare to share the savings achieved through the demonstration. Medicare will share 50 percent of the savings it gains under the demonstration with the Medicare beneficiary up to a maximum of $1,157. The exact amount of the shared savings payment will vary by site and procedure. Medicare will send the shared savings payment directly to qualified beneficiaries approximately 90 days after they are discharged from the hospital. (Those Medicare beneficiaries who are receiving Medicaid benefits are not eligible to receive shared savings payments.)

This hospital may share your clinical data with Medicare (Centers for Medicare & Medicaid Services and its contractors) for purposes of participating in the above-mentioned demonstration and for quality assurance related to the demonstration.
