Inherited Arrhythmias: What You Should Know About Genetic Heart Rhythm Disorders

Most heart rhythm disorders develop later in life as a result of aging, heart disease, or other medical conditions. Some, however, are caused by inherited genetic changes that affect the heart’s electrical system.

Although inherited arrhythmias are relatively uncommon, they can increase the risk of serious heart rhythm issues and, in some cases, sudden cardiac arrest. Because these conditions often run in families, recognizing a pattern of symptoms or learning about a relative’s diagnosis can be an important first step toward evaluation and treatment.

Understanding which heart rhythm disorders may be inherited and knowing when to seek medical care can help patients and families make informed decisions about their heart health.

What is an inherited arrhythmia?

An arrhythmia is any condition that causes the heart to beat too quickly, too slowly, or irregularly. Inherited arrhythmias occur when genetic changes affect the electrical signals that coordinate each heartbeat.

Unlike arrhythmias caused by coronary artery disease or structural heart problems, inherited arrhythmias can occur in people whose hearts are otherwise healthy. Some individuals experience symptoms during childhood or early adulthood, while others may not develop symptoms until later in life.

In some cases, a person may not realize they have an inherited arrhythmia until another family member is diagnosed.

Common inherited arrhythmias

Several inherited arrhythmias have been linked to genetic changes that affect the heart’s electrical system. Although each condition is unique, all require careful evaluation and individualized treatment.

Long QT syndrome affects the amount of time it takes the heart’s electrical system to reset between beats. This can increase the risk of ventricular arrhythmias, especially during exercise, emotional stress, or sudden excitement.

Wolff-Parkinson-White syndrome, often called WPW, occurs when an extra electrical pathway develops within the heart. This additional pathway can allow electrical signals to travel abnormally, which causes episodes of rapid heart rate that may begin and end suddenly.

Brugada syndrome is an inherited condition that affects the heart’s electrical signaling and may increase the risk of ventricular arrhythmias. Symptoms often occur at rest or during sleep, although many people remain unaware of the condition until it is identified through testing or family screening.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare inherited arrhythmia in which physical activity or emotional stress can trigger abnormal heart rhythms. Although uncommon, CPVT often appears in children and young adults and requires specialized management to reduce the risk of serious complications.

When should you talk with your physician?

Not everyone with palpitations or an irregular heartbeat has an inherited arrhythmia. However, certain symptoms and family history may suggest that additional evaluation is appropriate.

You should discuss your symptoms with a physician if you have experienced unexplained fainting, episodes of rapid heartbeat, or recurrent dizziness, particularly during exercise or emotional stress. A family history of sudden cardiac death, unexplained drowning, unexplained seizures, or relatives diagnosed with an inherited heart rhythm disorder should also be shared with your health care provider.

These factors do not necessarily mean you have an inherited arrhythmia, but they can help guide decisions about additional testing.

How are inherited arrhythmias diagnosed?

Diagnosing an inherited arrhythmia often begins with a detailed discussion about symptoms, personal medical history, and family history. An electrophysiologist may recommend an electrocardiogram (ECG), heart rhythm monitoring, exercise testing, or other specialized studies to better understand how the heart’s electrical system is functioning.

For some patients, genetic testing may also be recommended. When a genetic cause is identified, testing may help determine whether other family members could also be affected.

Treating inherited arrhythmias

Treatment for inherited arrhythmias depends on the specific condition, the severity of symptoms, and a patient’s overall risk level.

Some patients can successfully manage their heart rhythm disorder with medications or lifestyle modifications, while others may benefit from catheter ablation or an implantable cardioverter-defibrillator (ICD).

Because inherited arrhythmias can affect family members as well, it’s a good idea for close relatives to be screened for the same condition.

Understanding your risk starts with a conversation

Inherited arrhythmias are uncommon, but identifying them early can help guide treatment decisions and, in some cases, allow family members to be evaluated as well.
