Clinic Appointment
What to Bring:
- Insurance card(s)
- Current medications or an updated list of your current medications including name of medication, dosage and how often you take it.
- Name and phone number of your preferred pharmacy
- Previous medical records (if not already obtained by our offices)
- Copies of any previous angiograms, TEE, CT, MRIs or any other diagnostic imaging scans related to your condition(s) (if not already obtained by our offices)
Outpatient Procedure (Same Day)
What to Bring:
- Wear Clean Comfortable Clothing
- Insurance card(s)
- Completed Anesthesia Preoperative Questionnaire (if given one on a previous visit)
- Current medications and or an up-to-date list of current medications
- A change of clothes and toiletries in case of an overnight stay (you can leave these in the car)
Leave at Home:
- Valuables such as jewelry, watches and similar items
Inpatient Procedure (Overnight Stay)
What to Bring:
- Wear Clean Comfortable Clothing
- Insurance card(s)
- Completed Anesthesia Preoperative Questionnaire (if given one on a previous visit)
- Current medications and or an up-to-date list of current medications
- A change of clothes and toiletries for your stay (you can leave these in the car)
Leave at Home:
- Valuables such as jewelry, watches and similar items
