Now that you have an appointment with an OHH physician, let’s prepare for your upcoming visit. Complete the forms and bring them with you to your appointment to assist in your check-in process.

What to bring to a clinic appointment

  • Insurance card(s)
  • Current medications and or an up-to-date list of current medications
  • Name and phone number of your preferred pharmacy
  • Previous medical records (if not already obtained by our offices)
  • Any completed patient forms downloaded from this website
  • Copies of any previous angiograms, TEE, CT, MRIs or any other diagnostic imaging scans related to your condition(s) (if not already obtained by our offices)

What to bring for an outpatient procedure (likely go home the same day)

Wear clean comfortable clothing.

Bring:

  • Insurance card(s)
  • Completed Anesthesia Preoperative Questionnaire (if given one on a previous visit)
  • Current medications and or an up-to-date list of current medications
  • A change of clothes and toiletries in case of an overnight stay (you can leave these in the car)

Leave at home:

  • Valuables such as jewelry, watches and similar items

For an inpatient procedure (overnight stay involved)

Wear clean comfortable clothing.

Bring:

  • Insurance card(s)
  • Completed Anesthesia Preoperative Questionnaire (if given one on a previous visit)
  • Current medications and or an up-to-date list of current medications
  • A change of clothes and toiletries for your hospital stay

Leave at home:

  • Valuables such as jewelry, watches, and similar items
